Qualifications

• RN license required.
• Minimum nursing diploma or associate's degree.

• Educates patients and families on illness management and post-treatment home care.
• Ensures patient safety and quality through ongoing professional development.
• Plans and communicates the care plan across the care continuum with the team.
• Assesses patient needs and coordinates with interprofessional team including family.
