Job summary

Daybreak Independent Services, Inc. is seeking a Quality Assurance Investigator to conduct timely, objective investigations into incidents and regulatory concerns within our programs. The role ensures compliance with OPWDD standards and supports safety, transparency, and continuous improvement.

Gathers evidence, interviews involved parties, prepares investigative reports, and recommends corrective actions to strengthen practices. Full-time position with travel between program sites.

Qualifications

• Bachelor's Degree required; OPWDD experience preferred.
• Good verbal and writing skills; experience in Developmental Disabilities settings valued.

Responsibilities

• Participates in in-service trainings and on-site workshops.
• Investigations must follow NYS OPWDD Parts 624 and 625 regulations.
• Prepare interviewed questions and gather documentation for investigations.
