A Brief Overview
The Nurse Navigator serves in an expanded nurse's role spanning across the health continuum with a focus on effective and safe transitions through the healthcare system. The Navigator functions as a coordinator for patients through an episode of care for treatment of back pain and other designated pain. The Navigator collaborates with surgeons, primary care providers, rehabilitation specialist(s), and other health team members to provide high-quality care for patients following established standards and practices. The Nurse Navigator guides patients through the health care system by assisting with access issues, developing relationships with service providers, and tracking interventions and outcomes. The Nurse Navigator acts as a coordinator of patient care, assesses, plans, implements, monitors, and evaluates all options and services with the goal of optimizing the pain management patient outcomes. The Nurse Navigator integrates evidence-based clinical guidelines, preventative guidelines, protocols, promoting quality and efficiency in the delivery of healthcare. The delivery of professional nursing care at Stormont-Vail Health is guided by Jean Watson's Theory of Human Caring and the theory of Shared governance, both of which are congruent with the mission, vision, and values of the organization.
Education Qualifications
- Bachelor's Degree Bachelor's of Science in Nursing (BSN) Preferred
Experience Qualifications
- 3 years Professional Nursing experience, or directly related, experience in acute care or outpatient clinic setting. Required
- 3 years Case/care management experience. Preferred
Skills And Abilities
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to read, analyzes, and interprets general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to effectively present information and respond to questions from physicians, patients, co-workers and the general public. (Required proficiency)
- Professional nursing theory and practice to give and evaluate patient care:
- Medical equipment and instruments to administer patient care
- Common safety hazards and precautions to establish a safe work environment (Required proficiency)
- Ability to define problems collects data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions in the mathematical diagram form and deal with several abstracts and concrete variables. (Required proficiency)
- Applying and modifying the principles, methods and techniques of professional nursing to provide on-going patient care. (Required proficiency)
- Developing and maintaining department quality assurances. (Required proficiency)
- Establishing and maintaining effective working relationships with patients, medical staff and the public. (Required proficiency)
Licenses and Certifications
- Registered Nurse - KSBN Required
- Basic Life Support - BLS Required within 90 days of hire.
What you will do
- Guides patient throughout Pain Management pathways and healthcare system.
- Initiates communication with patients upon learning they are seeking interventions or treatment for pain.
- Insures appropriate testing and follow-up are complete on external patients returning to our system.
- Facilitates interaction and communication with health care staff providers.
- Completes assessments and ensures necessary diagnostic work is complete prior to appointments and / or procedures.
- Provides disease/treatment specific education to both patients and families.
- Verify benefits and obtain authorizations. Contact insurance companies to conduct and complete benefit investigations.
- Maintains timely flow of patients. Maintains and reviews patient records.
- Triages and refers incoming telephone calls.
- Supports shared decision making.
- Conducting initial patient intakes (collecting patient information, insurance information, referral source, brief relevant clinical information, etc.)
- Delegates tasks and duties in the direction and coordination of health care team members, patient care and department activities in accordance with the Kansas State Nurse Practice Act.
- Self-directed and purposeful in seeking the necessary knowledge and skills to enhance professionalism in order to provide high-quality care.
- Participates in system wide/department patient care quality improvement activities as well as standards development
- Demonstrates knowledge of the Kansas State Nurse Practice Act and follows the statutes and regulations therein
