Job summary

Henry Ford Health is seeking a Registered Nurse to implement and monitor patient care plans, coordinate disciplines, and communicate patient status to the care team. The role serves as the primary coordinator for well-coordinated patient care and ensures orders are carried out by the team.

Candidates must hold a Michigan RN license prior to hire, have a diploma from an accredited nursing program, and obtain BLS within 1 month of hire. AHA or ARC certification is accepted.

Qualifications

• Diploma from an accredited school/college of nursing or required professional licensure at time of hire.
• Registered Nurse credentialed from the Michigan Board of Nursing obtained prior to hire date or job transfer date.

Responsibilities

• Implements and monitors patient care plans.
• Monitors, records and communicates patient condition as appropriate.
• Serves as a primary coordinator of all disciplines for well-coordinated patient care.
• Notes and carries out physician and nursing orders.
• Assesses and coordinates patient discharge planning needs with the healthcare team.
