Job summary

Northwell Career Site seeks a Registered Nurse to deliver age-specific care using the nursing process. You will assess, plan, implement and evaluate patient care, collaborating with physicians and the health team to coordinate treatments on a designated unit.

Responsibilities include physiologic and psychosocial assessments, care planning, medication administration, patient education, discharge planning and documenting outcomes to support safe, effective care.

Qualifications

• Graduate from an accredited School of Nursing.
• Minimum 1-2 years of professional nursing experience in the related specialty.
• Current license to practice as a Registered Professional Nurse in New York State, required.
• BLS required, plus specialized certifications as needed.

Responsibilities

• Assesses the patient’s physiologic health status and documents findings.
• Assesses patient’s psychosocial health status and caregiver needs.
• Identifies patient outcomes and develops individualized goals.
• Formulates and prioritizes the plan of care and coordinates resources.
• Implements the plan of care, monitors safety, and documents care.
• Evaluates care effectiveness and outcomes for continuous improvement.
• Collaborates with the care team and delegates appropriately.
• Performs Decubitus care and wound/skin assessments.
• Administers medications safely and educates patients about drugs.
• Participates in patient/family education and discharge planning.
• Performs related duties as required.
