Job summary

Memorial Health in Decatur, IL is seeking a Nurse Practitioner or Physician Assistant to join the Interventional Radiology Team at Decatur Memorial Hospital. This is a Full Time Day Shift Advanced Practice Provider role with strong patient care responsibilities and collaboration with the IR physicians.

The role includes care coordination, education, and referrals, with licensure as APRN or PA and CPR (ACLS where required).

Qualifications

APRN or PA licensed per Illinois law; DEA and Controlled Substances licenses required within 3 months if not held at time of hire.
CPR certification required; ACLS where required.
Minimum 1 year post-graduate mid-level provider experience preferred.

Responsibilities

Provide evidence-based care and evaluation; coordinate department care under physician direction.
Collaborate with physicians and other healthcare staff; refer patients as needed.
Document all care in electronic medical record and perform routine exams and health maintenance.
Teach wellness habits, educate patients, and develop practice education programs.
Coordinate patient care and family counseling with public and outside agencies as appropriate.
