Job summary

Highmark Health is seeking an RN Nurse Navigator in Pittsburgh, PA to serve as a consistent point of contact for physicians, patients, caregivers, and the community, coordinating care across disease states and ensuring smooth patient flow.

The role requires 1–3 years of nursing experience, a Bachelor's in Nursing preferred, and active PA RN license (and related clearances). This full-time, on-site position offers opportunities to impact outcomes and collaborate with a multidisciplinary team.

Qualifications

• Bachelor's Degree in Nursing preferred.
• 1-3 years nursing experience in the specialty required.
• Current PA RN licensure or multi-state licensure via eNLC.
• CPR – American Heart Association.
• Act 34 Criminal Background Clearance Certificate.
• Act 33 Child Abuse Clearance Certificate.
• Act 73 FBI Fingerprinting Criminal Background Clearance Certificate.
• Valid PA driver's license may be required.
• OAPSA clearance may be required depending on location.

Responsibilities

• Oversee community outreach and establish relationships within the network.
• Educate and coordinate care regarding patients' diagnoses and treatment options.
• Ensure safe handoff and coordination of care between facilities.
• Work with High-Risk Care Teams to reduce re‑admissions.
• Assist Registry staff with data collection and patient outcomes.
• Coordinate appointments with the multi-disciplinary team and accompany patients as needed.
• Address medication adherence issues.
• Identify at‑risk patients and facilitate appropriate screening processes.
• Trend data and outcomes for the navigation program.
• Develop or attend clinical care conferences to report on active patients.
