Department: 12801 Advocate Aurora Health Corporate - Trauma Recovery Center: Condell Hospital Status: Full time Benefits Eligible: Yes Hours Per Week: 40 Schedule Details/Additional Information:

• Onsite role at Advocate Condell Medical Center - 801 S Milwaukee Ave
• primarily 2nd shift/overnight, but can vary depending on needs
• Coverage as needed including days/weekends/holidays
• Tues/Weds/Thurs/Friday, every third Saturday for 8 hour shift, 6pm-2am / 7pm-7am
• Shift differential

Pay Range: $22.90 - $34.35 The Lead supports leadership by providing onsite guidance and support to the team responsible for inpatient care for individuals who have experienced recent trauma. This role assists with on-call schedules, fosters collaboration with internal and external stakeholders, coordinates hospital and trauma clinic schedules, ensures data quality, monitors productivity, and fulfills other key responsibilities under the direction of the leadership team. Major Responsibilities: Team Navigation: Assists the leadership with daily operations and team performance by guiding the team to meet individual and team goals, high standards of productivity, quality documentation, interdisciplinary collaboration, and fostering a supportive work environment. Patient Support: Provides immediate emotional support and resources to hospitalized patients following a trauma, including connecting them with the clinical team when appropriate, assisting with discharge planning, leading support and psychoeducation groups, and safety planning. Also, complete assessments and facilitate warm-handoffs to providers for ongoing care. Resources Navigation: Responds to trauma alerts, safety calls, pre-escalation needs and collaborates with street outreach teams and facilitates mediation to mitigate retaliation risks. Outreach: Guides the team, offers patient support, works alongside other groups, and actively engages in intensive community outreach. This also involves making follow-up telephone calls and sending letters to discharged patients to connect them with outpatient resources. Continuity of Care: Schedules patient appointments with the TRC, other internal Advocate Health resources, or community partners before discharge and provides brief follow-up calls after discharge when necessary, to ensure ongoing care which requires strong collaboration with other providers. Assessments: Conducts intakes and needs assessments, linking patients to appropriate resources and outpatient services. It may also require notifying other hospital providers of unmet needs to be addressed while in the hospital. Record Keeping: Maintains accurate records of patient interactions and action plans. Collaboration: Works with the hospital treatment team and community partners to ensure comprehensive treatment, discharge planning, and continuity of care. Minimum Job Requirements Education
