Job summary

LaSante Health Center in Brooklyn seeks an Internal Medicine Provider to deliver comprehensive primary and preventive care to adult patients, manage chronic conditions, and promote wellness in a culturally diverse community. Creole-speaking candidates are strongly preferred.

You will collaborate with multidisciplinary teams, document thoroughly in the EHR, participate in quality improvement initiatives, and support patient-centered care.

Qualifications

• MD, DO, NP, or PA credential from an accredited institution.
• Board certification or eligibility in Internal Medicine or Adult Primary Care.
• Current New York State license or licensure eligibility.
• Experience in community health or outpatient primary care preferred.
• Creole/English bilingual abilities strongly preferred.

Responsibilities

• Provide primary and preventive care for adult patients, including physical exams, screenings, and chronic disease management.
• Diagnose and treat acute and chronic medical conditions.
• Collaborate with multidisciplinary teams to coordinate patient-centered care.
• Counsel patients on lifestyle modification, disease prevention, and medication management.
• Maintain accurate, timely documentation in the EHR.
• Participate in quality improvement initiatives and team meetings.
• Complete additional tasks as directed by supervisor.

The ideal candidate will be committed to delivering comprehensive primary and preventive care to adult patients, managing chronic conditions, and promoting overall wellness in a culturally diverse community. Responsibilities:

• Provide primary and preventive care for adult patients, including physical exams, screenings, and chronic disease management
• Diagnose and treat acute and chronic medical conditions
