Qualifikationen

• Medical degree from an accredited institution (MD or DO).
• Board Certification or Board Eligibility in Internal Medicine.
• Active New York Medicaid provider number — required.
• Strong clinical acumen across a broad range of adult medical conditions.
• Warm, engaging personality with a genuine passion for serving diverse communities.
• Ability to work effectively in a high-volume outpatient telehealth environment.
• Excellent interpersonal and communication skills.
• Demonstrated ability to collaborate within a team-based, multi-specialty care model.

Aufgaben

• Provide comprehensive internal medicine care via telehealth, conducted on-site from the Brooklyn practice location.
• Manage a broad range of acute and chronic medical conditions including diabetes, hypertension, cardiovascular disease, and related disorders.
• Conduct virtual patient evaluations, develop individualized treatment plans, and ensure appropriate follow-up care.
• Collaborate with an integrated in-house team of specialists across more than 30 departments.
• Coordinate internal referrals and ensure continuity of care for patients with complex medical needs.
• Educate patients on disease management, preventive care, and healthy lifestyle practices.
• Maintain thorough, accurate, and up-to-date medical documentation in accordance with established standards.
• Adhere to all applicable Medicaid, regulatory, and ethical standards of medical practice.
