1:40). The Caldwell Health Plaza hosts a wide range of services, targeting comprehensive care for the community with modern amenities and onsite diagnostics.

Qualifications

Saint Alphonsus Medical Group (SAMG)is a physician led and accountable 650+ provider multi-specialty group practice that has anexcellent opportunity for a full-time or part-time Internal Medicine Physician (MD/DO)to join a group providers in a brand-new, 60,000+ square foot facility. We are seeking a new physician for this location and would welcome residents. The schedule for this opportunity is as follows: For full-time - 34 patient contact hrs., with option for 4- or 5-day work week. The call requirement is Practice call shared with other eligible FM providers (Appx 1:40).

The Caldwell Health Plazaopened last fall and is now a vibrant, fully operational, state of the art 60,000+ sq ft facility. The Health Plaza serves the community members seeking wellness, diagnosis, treatment, and rehabilitation in a convenient and accessible environment, located directly off I-84. With comprehensive healthcare services offered for the whole family, it offers a multitude of services in one convenient location including: Extended Hours Urgent Care, Family Medicine, Pediatrics, Occupational Medicine, Diabetes Care & Education, Sleep Medicine, Orthopedics, Heart Care, Physical Therapy, Occupational Therapy, Podiatry, MFM, ENT and onsite Lab, X-ray, and ultrasound.

Requirements:

• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree
• Successful completion of an accredited ACGME/AOA Internal Medicine residency program
• Board certified, or obtain such certification within 3 years of employment
• Obtain Idaho medical licensure to practice medicine in all its phases
• Valid controlled substance registration with Idaho Boards of Pharmacy & DEA.
