Job summary

Northshore Home Health Care is seeking a Board-eligible or board-certified Internal Medicine physician for a full-time outpatient role in a Patient-Centered Medical Home.

You will deliver team-based, patient-centered care to adult patients in an outpatient setting, with a mix of in-person and virtual visits. Participation in shared phone call coverage and collaboration within a multidisciplinary team is expected. Teaching and research opportunities are available, with optional urgent care.

Qualifications

• MD/DO with board eligibility or board certification in Internal Medicine.
• Strong communication and patient engagement skills across in-person and virtual formats.
• Ability to work effectively in a multidisciplinary team environment.
• Commitment to teaching and quality improvement activities.
