Tapestry 360 Health is seeking a passionate and dedicated Physician to join our mission-driven team, committed to providing quality, culturally competent primary care to underserved communities. Our physicians play a pivotal role in enhancing community health through accessible, prevention-oriented, and collaborative healthcare services.

Essential Duties and Responsibilities
  • Patient Care: Conduct comprehensive health assessments, including physical examinations, ordering diagnostics, and developing individualized treatment plans.
  • Prevention and Treatment: Emphasize disease prevention, health maintenance, and holistic patient wellness across all age groups.
  • Collaborative Practice: Work in a team-based care model with nurse practitioners, physician assistants, and allied health professionals, ensuring cohesive and integrated care.
  • Community Focused: Engage in outreach, patient education, and preventive care initiatives to reduce health disparities within the community.
  • Documentation: Accurate documentation of all patient interactions, diagnoses, treatments, and follow-up care in an electronic health record (EHR) system.
  • Quality Improvement: Participate in quality improvement initiatives, peer review, and evidence-based practice updates to continuously elevate patient care standards.

Clinical Expertise in Primary Care

Qualifications

Required Qualifications
  • Education: Doctor of Medicine (MD) or Doctor of Osteopathy (DO) from an accredited medical school.

    • Documentation of completed residency.
  • Experience: Proven ability to manage both common and complex health conditions within a community health setting. Experience with underserved populations is highly desirable.

    • Ability to work effectively within a multidisciplinary team.

Pay Transparency

The salary range for this position is $160,000 – $200,000 annually. Actual compensation will be determined based on factors including, but not limited to, relevant experience, qualifications, internal equity, and organizational needs.
