Job summary

Theoria Medical is seeking an Advanced Practice Provider for full-time work in Ossipee, NH, delivering patient-centered care in skilled nursing facilities. You’ll provide rounds, prescribe treatments, and collaborate with facility staff to ensure continuity and quality of care.

Qualifications

Responsibilities

• Conduct daily rounds and develop/adjust treatment plans.

What You'll Do

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.

Your Qualifications

Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
