Job summary

Theoria Medical in Sulphur, LA seeks a PRN Advanced Practice Provider to deliver high-quality, patient-centered care in skilled nursing facilities, with flexible scheduling and clinician autonomy.

You will round, prescribe and manage treatment plans, liaise with facility staff, and ensure regulatory compliance, supported by an intuitive EHR, AI Scribe, and UpToDate for guidance.

Qualifications

• Active U.S. license and board certification by ANCC, AANPCB, PNCB, or AACN (Nurse Practitioners) or NCCPA (Physician Assistants).
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Geriatric Medicine.

Your Qualifications

Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
