Job summary

Theoria Medical is seeking a Part-Time Advanced Practice Provider to care for residents in skilled nursing facilities nationwide. You will join a mission-driven team that values relationships, continuity of care, and flexible scheduling while using intuitive technology to support daily patient care.

Responsibilities include daily rounds, prescribing medications, and addressing acute issues, with strong emphasis on documentation, regulatory compliance, and collaboration with facility staff.

Qualifications

• APPs must hold an active U.S. license and be board-certified by one or more approved boards.
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, geriatrics.
• Nurse Practitioners or Physician Assistants eligible under the listed boards.

What You'll Do

• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.

Your Qualifications

Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
