Job summary

Ascension’s Joshua Max Simon Primary Care Center in Indianapolis seeks an NP or PA to provide outpatient internal medicine services. The role features full-time hours with a mix of clinical and administrative duties, supporting preventive and chronic care in a busy ambulatory setting.

This employed position offers access to Athena EMR, Sunrise hospital reference, 19 exam rooms, and translation/interdisciplinary collaboration to deliver high-quality patient care within Ascension’s mission-driven

Qualifications

• Active Indiana RN license and NP/PA licensure or eligibility.
• BLS Provider certification within 30 days of hire date.
• Nurse Practitioner or Physician Assistant credential with Prescriptive Authority from Indiana Board of Nursing.
• Master's degree in nursing required.

Responsibilities

• Deliver preventive, acute, and chronic outpatient care in a 100% ambulatory setting.
• Collaborate with a multidisciplinary care team to coordinate patient care.
• Provide patient education and follow-up to improve health outcomes.
• Utilize translation services and on-site/remote interpretation as needed.
• Maintain timely and accurate documentation in the Athena EMR system.

How you’ll make an impact in this role

• Join an established team of 8 Internal Medicine physicians
• Call structure: None
• Practice setting:Outpatient (100% ambulatory care; 6-7 appointment slots per half-day session across 9 sessions/week)
• Access to Athena EMR (with Sunrise for hospital reference), 19 exam rooms (including one minor procedure room), on-site/remote interpretation services, and real-time physician collaboration
• Employed position within a nonprofit health system
• Board Certified or Board Eligible Nurse Practitioner (NP) or Physician Assistant (PA)
• Eligible for state medical licensure and DEA registrationEligible for Indiana state medical licensure and DEA registration
• Independently manage a comprehensive outpatient Internal Medicine schedule, delivering preventive care, acute visit management, chronic disease care, and post-hospital discharge follow-ups.
• Provide high-quality, compassionate care to a mission-focused, culturally diverse, and medically/socially complex patient population, collaborating with multidisciplinary care teams and utilizing translation services.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Nurse Practitioner specializing in Prescriptive Authority credentialed from the Indiana Board of Nursing obtained within 6 Months (180 days) of hire date or job transfer date required.
• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.

Education

• Master's degree of Nursing required.
