Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required
• Federal DEA and Illinois Controlled Substance License required
• Education: Completion of master's level program in specialty area
• Certification: BLS/CPR certification required
• Experience: 2 years APN/PA experience preferred

Responsibilities

• Provides care to patients under the direction of collaborating physicians
• Obtains detailed history and performs physical examinations
• Orders imaging, lab tests and diagnostic procedures and interprets results
• Instructs patients on prescribed therapies and pre/post operative instructions
• Maintains records and writes progress notes
• Prescribes pharmacologic and non-pharmacologic therapies as appropriate
• Collaborates with physicians and health care team to achieve optimal outcomes
• Promotes wellness and preventive care across settings
• Works autonomously and in interdisciplinary teams as resources
• Supports organizational goals and adheres to Endeavor Health policies
