Job summary

Integrated Services for Behavioral Health is offering an Intern – Community Health Worker (CHW) Field Placement. The intern will support outreach, education, and supportive services under supervision, engaging with clients and families across the lifespan.

Responsibilities include coordinating outreach, liaising with health and social service organizations, and educating communities on health topics and resources. This internship emphasizes service, teamwork, and community impact.

Qualifications

• Recently completed CHW certificate program - REQUIRED.
• Interest in community health, health equity, social determinants of health.
• Strong communication, empathy, and cultural competency.

Responsibilities

• Assist in outreach to individuals and families to identify health and social service needs.
• Serve as a liaison between community members and health/social service organizations.
• Educate clients about health topics, preventive care, resources, and referrals.
• Help with coordination, scheduling, and tracking client progress.
• Participate in team meetings, training, and supervision sessions.
• Support non-clinical tasks (e.g. health promotion events, workshops, community engagement).
