Job summary

Sweetwater Care in California is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care within our Intermediate Care Facilities for individuals with disabilities. You will implement health care plans and administer medications for residents in small, close-knit homes.

Collaborating with a multidisciplinary team, you will support Direct Support Professionals, maintain medical records, and participate in ongoing learning opportunities to ensure high-quality care and compliance

Qualifications

Facility Information:

• Our facilities are Intermediate Care Facilities for individuals with disabilities (ICF/DD-H and ICF/DD-N)
• Caregiver (DSP) to people supported ratio is usually 1:3, sometimes as high as 1:1 support
• Nursing caseload typically ranges from 6-15 residents
• Our home sizes are small, including 6-bed and 15-bed facilities
• High-acuity care is limited or prohibited by facility regulations
• Our Immediate Care Facilities provide wound care per licensing requirements, ranging from simple wounds to a maximum of stage I pressure ulcers
• Hospice care is very limited and is provided collaboratively with professional hospice care providers who come to our homes

Licensed Vocational Nurse (LVN) Essential Responsibilities:

• Provide skilled nursing care to individuals with disabilities in an Intermediate Care Facility setting
• Implement and oversee the health care plans for residents
• Administer medications and perform medical procedures as neededCollaborate with a multi-disciplinary team to ensure the well-being of residents

• Offer support and guidance to Direct Support Professionals (DSP)

Licensed Vocational Nurse (LVN) Qualifications:

• Valid License Vocational Nurse (LVN) license in the state of California
• CPR certification
• Strong communication and interpersonal skills
• A genuine passion for providing quality care and support to individuals with disabilities
• Ability to work effectively in a team-oriented environment
• Willingness to adapt to the needs of the residents and facility
