Job summary

You will oversee patient care transitions, provide leadership to nurse managers, and collaborate with medical staff to ensure high-quality, compassionate outcomes.

Minimum five years of related experience with at least three years in a director or manager role, and licensure in the state of placement are required; a Master’s degree is preferred.

• Licensure in the state of placement required.
• Compact Licensure preferred; with the ability to obtain licensure in other states as needed.

RESPONSIBILITIES & JOB FUNCTIONS

The Leader assumes responsibility for care of patients as they move through the areas of responsibility and into the home or other level of care in the community.

Provides leadership and support to nurse manager(s) using creative leadership to ensure best outcomes in high quality compassionate patient care.

The Director works with medical staff and other health care professionals collaboratively.

EDUCATION & EXPERIENCE

• Bachelor’s degree required, Master’s degree preferred

LICENSURE & CERTIFICATIONS
