• Eight years of clinical experience and five years healthcare leadership experience required; acute care case management experience preferred.

Responsibilities

• Provide strategic and operational leadership for acute care case management across two hospital sites.
• Oversee four direct reports and 44 FTEs.
• Lead case management operations and improve length-of-stay performance.
• Collaborate with Utilization Management and Revenue Cycle and ensure patient-centered care coordination.
