Job summary

Ahava Hospice is seeking a dedicated Chaplain to join our compassionate care team. You will provide direct spiritual care to patients and families, conduct assessments, and help develop the plan of care aligned with each person's beliefs and values.

You will participate in interdisciplinary meetings, educate staff on spiritual needs, recruit local chaplains, and offer memorial services as requested, supporting our mission to provide loving, person-centered end-of-life care.

Qualifications

• Graduate of accredited seminary or school of theology (BD, MDiv. or equivalent theological degree).
• Ability to work as member of interdisciplinary group.
• Comfort in an interfaith setting.
• Ability to accept different lifestyles, cultures, beliefs, and values.
• Ability to network with community clergy and congregations.
• Knowledge of and commitment to hospice philosophy of care.
• If making home visits, must be a licensed driver with insured automobile.

Responsibilities

• Assures spiritual assessment of patients and families/caregivers in the hospice program.
• Facilitates development of individualized plan of care by participating in comprehensive assessment.
• Provides direct spiritual care to patients and families/caregivers.
• Serves as liaison to community chaplains and spiritual counselors.
• Maintains records of spiritual care services for quality assurance and program development.
• Attends care conferences as a member of the interdisciplinary group.
• Provides consultation, education, and support to the interdisciplinary group on spiritual care.
• Recruits community chaplains and spiritual care volunteers to meet patient needs.
