Intensive Case Manager Bilingual in English and Spanish (Mount Dora and Orange County)
This position provides high-quality, client-centered, intensive supportive services to clients in the Ryan White (RW) program. Providing RW case management services, including assessing client’s needs, conducting psychosocial evaluations, developing individual treatment plans, and maintaining documentation of services and staff activities. It also includes providing education and support to clients, making appropriate referrals and consultations both within and outside the Agency. These case management services will monitor all aspects of the client’s care in the client records to the standard of Miracle of Love (MOL).

Position Responsibilities
- Provide a wide range of client-centered services that link clients to health care, psychosocial and other services to ensure timely, coordinated access to medically appropriate levels of care and support services.
- Support continuity of care, ongoing assessment of the client and other family members' needs, and personal support systems that prevent unnecessary hospitalizations or expedite discharge, as medically appropriate, from inpatient facilities.
- Meet services productivity (direct client services) standard of 560 units per month for the provision of Case Management services in accordance with applicable quality criteria, supported by appropriate record documentation.
Key Intensive Case Manager Activities include:
- Provide intensive case management services with clients including assistance with receiving appropriate mental health/substance abuse treatment, applying for benefits, and establishing/maintaining housing.
- Maintain a tentative caseload of 35 individuals.
- Maintain all required and appropriate documentation for client records and progress notes for assigned cases in accordance with Miracle of Love, Inc. policy and procedures.
- Maintain weekly contact with clients with a minimum of two (2) face-to-face engagements per month until the client is virally suppressed.
- Conduct initial comprehensive assessment of the client’s needs and personal support systems.
- Develop a comprehensive, individualized service plan that is completed with the client.
- Coordinate the services required to implement the plan.
- Monitor client to assess the efficacy of the plan.
- Coordinate patient prescription distribution with local pharmaceutical companies or complete the referral for ADAP with clients that meet program requirements.
- Develop and maintain working relationships with other agencies including the Corrections/Jail staff and officials, Boards of Mental Health, County Attorney and Public Defenders offices, and other agencies to assist with providing services for clients.
- Make professional decisions, recommendations, and maintain confidentiality as stated in the MOL Confidentiality Agreement.
Essential Skills and Experience:
- Florida Department of Health 500 and 501 HIV/AIDS certification (if not completed, MOL will provide this resource during training process).
- Bachelor’s Degree in Social Work or Public Health Field Preferred.
- Clean criminal history.
- Ability to complete assessments and develop plans for individual cases.
- Ability to follow through on all phases of assigned treatment plans for clients.
- Ability to communicate effectively verbally and in writing.
- Direct Care Case Management experience.
License: Must possess a valid Florida driver’s license or have access to reliable transportation.

- Physical demands: While performing the duties of this job, the employee is occasionally required to walk, sit, use hands to handle or feel objects, reach with hands and arms, talk or hear. The employee must occasionally lift and/or move up to 15 pounds.
- Work environment: While performing the duties of the job, the employee is exposed to bodily fluids and weather conditions prevalent at the time of duty.
Starting salary $43,000
