NARA Northwest is seeking an Intensive Outreach Therapist to deliver outreach, intake and mental health assessments in Portland, OR. The role focuses on clients who are homeless or have been homeless, providing individual and group therapy and aiding transitions toward employment services.

The position emphasizes collaboration with housing staff, documentation, crisis intervention, and ongoing client supports in a community-based setting with some office hours.

Qualifications

• Master’s degree in Social Work, Psychology, Counseling, or related field.
• Certified QMHP in Oregon through MHACBO and/or associate license with corresponding board.
• Meets QMHP requirements per Oregon Administrative Rules.
• Ability to maintain professional boundaries with clients, staff, and community.
• Reliable transportation with clean driving record and proof of auto insurance.
• Must pass pre-employment drug tests and background checks.

Responsabilités

• Provide intakes and mental health assessments for clients in supportive housing.
• Deliver individual and group therapy sessions.
• Assist clients in pursuing employment services when appropriate.
• Coordinate with housing staff to provide supports, advocacy, and problem-solving.
• Serve as liaison with referral sources and other staff.
• Complete diagnostic assessments, treatment plans, progress notes, and discharge summaries on time.
• Provide crisis intervention as needed and assist with follow-up.
• Conduct case management, referrals, follow-ups, and outreach as required.
