Qualifications

• Experience providing assessments and treatment planning.
• Ability to conduct individual, family and group therapy.
• Ability to plan discharge and coordinate care across teams.

Responsibilities

• Provide direct clinical services per accepted practice standards.
• Plan and conduct group therapy as directed.
• Perform Comprehensive Integrated Assessments and interpret results.
• Develop Multidisciplinary Treatment Plans under physician supervision.
• Engage in case management and discharge planning.
• Participate in multidisciplinary team meetings and report progress.
• Deliver care with ethical, respectful and culturally sensitive practice.
• Identify and address age/disability/cultural needs with patients and families.
• Involve support persons in decision-making as appropriate.
• Contribute to additional duties and projects as assigned.
