Job summary

Unique Caring is seeking an Intensive In-Home Associate Professional to provide direct, structured interventions to children, adolescents, and families within IIH services. Work under a Qualified Professional and LP oversight, delivering services in home, school, and community settings.

Key duties include implementing PCP, behavioral interventions, crisis support, family coaching, care coordination, and thorough documentation to meet NC Medicaid IIH requirements.

Qualifications

• Bachelor’s degree in a human services field or equivalent with relevant experience.
• Experience must be with MH/DD/SAS population.
• Associate degree in a human services field with experience.

Responsibilities

• Direct service delivery to children, adolescents, and families.
• Implementation of Person-Centered Plans (PCP).
• Behavioral intervention & skill building.
• Crisis support when needed.
• Family support & coaching.
• Care coordination across services and settings.
• Documentation & compliance with regulations.
• Team participation within the clinical team.
