Job summary

World Relief in the United States is seeking a dedicated Client Services role to support vulnerable populations. You will plan individualized service goals, conduct home visits, and coordinate with Medicaid-funded and community resources to meet client needs.

This position requires a mature Christian faith, strong organizational and communication skills, and the ability to work independently with flexible hours. Travel to client homes and appointments is expected.

Responsibilities

  • Create client service plans with goals for independence within the program period.
  • Coordinate with Medicaid-funded clients and perform necessary assessments.
  • Conduct home visits to assess ongoing needs and progress toward goals.
  • Provide orientation on topics such as financial literacy, parenting, and health navigation.
  • Identify medical providers and coordinate appointments to meet urgent needs.
  • Assist clients with health insurance navigation and medical bill payment.
  • Arrange transportation to medical and related appointments to promote independence.
  • Coordinate referrals to community services when needed.
  • Ensure smooth exit from intensive services with referrals as appropriate.
  • Coordinate with outside agencies to meet client needs and advocate for clients.
  • Collaborate with primary care and specialists to ensure timely medical care.
  • Participate in team case planning and program management meetings.
  • Prepare and submit monthly reporting and attend required trainings and meetings.
  • Complete forms and write case notes in mandated systems.
  • Maintain accurate client records and support documentation.
  • Assist in grant reporting with drafting of monthly stories.
  • Travel to client homes and other service providers as required.

ORGANIZATION SUMMARY

World Relief is a global Christian humanitarian organization whose mission is to boldly engage the world’s greatest crises in partnership with the church. Today, organizational programming focuses on humanitarian and disaster response, community strengthening and resilience, and refugee & immigrant services and advocacy.

ROLE & RESPONSIBILITIES:

Client Services

  • Create individual client service plans with specific goals leading to client independence to be achieved within the program period. The caseload at any given time will be approximately 20 individuals, from a variety of funding sources.
  • Work with individuals funded through Medicaid to provide case management supportive services. For Medicaid funded activities, conduct all necessary assessments and follow up through IM+CANS.
  • Conduct home visits and assess ongoing needs to assist the client in meeting service plan goals
  • Provide orientation around various topics, while assessing life skills in particular areas, i.e., financial literacy, parenting, mental health, navigating the health care systems, digital literacy, etc.
  • Identify specialist medical providers and coordinate client appointments to ensure that medical needs are met in a timely manner
  • Assist clients to navigate health insurance requirements for accessing medical specialist care and educate clients on how to work with health insurance to pay medical bills
  • Provide transportation to medical and medical-related appointments, assisting clients to become independent in navigating appointments
  • Assist clients to arrange for transportation to follow-up appointments and establish future transportation plans
  • Make appropriate referrals to community agencies for services not provided internally
  • Effectively exit clients from intensive service provision at the conclusion of the program period, including referrals to internal departments and/or outside entities

Planning, Reporting, and Coordination

  • Coordinate with outside agencies/organizations to meet client needs, advocating and mediating on behalf of clients, as needed
  • Coordinate with primary care providers, specialist medical providers, and medical insurance providers to ensure that clients receive appropriate, timely medical care
  • Coordinate with other WRDA teams and departments to provide strong collaborative care
  • Participate in weekly case planning meetings with direct service staff and program management staff
  • Ensure monthly reporting information is submitted in a timely and accurate manner, and all Preferred Communities (PC) program requirements are met.
  • Attend PC monthly meetings along with required training or HO annual conferences.

Administrative Responsibilities

  • Complete program-required forms within specified time frames, ensuring that forms are accurately recorded and filed
  • Write detailed case notes for each task/client interaction. Keep track in two different systems based on client service. Ritten is used for Medicaid and Dynamics for everything else
  • Retrieve the signatures of each participant and complete all necessary program paperwork
  • Write monthly stories and provide input for grant reports
  • Research topics and gather information for group sessions and the effectiveness while utilizing evidence-based practices and peer reviewed materials
  • Maintain accurate records of client files, case notes, new providers, use of interpretation, and reporting records
  • Actively participate in department meetings, staff development opportunities, program trainings, community activities, and organizational events
  • Regular travel is required. This includes travel to client’s home and aiding in the connection to other service providers and group sessions
JOB REQUIREMENTS:

  • Valid drivers license, access to reliable vehicle and ability to pass MVR standards
  • Cross-cultural experience and/or knowledge of the refugee/immigrant experience
  • Highly self-motivated with strong follow-through and ability to work independently
  • Strong mediation and problem-solving skills
  • Strong organizational skills and ability to maintain detailed, accurate records
  • Strong interpersonal and verbal communication skills
  • Strong Microsoft Office skills required
  • Ability to work a flexible schedule including occasional early mornings, evenings, and/or weekends
PREFERRED QUALIFICATIONS:
  • Bachelor’s degree or higher in social work, public health, or related field preferred
  • Fluency in Dari, Russian, Arabic, French, or Spanish is highly desired
  • Able to supervise interns is preferred
  • Comfortable working in a team environment
  • Direct-service experience with vulnerable populations preferred

$23.75 - $27.33 an hour
