Job summary

Directions for Living in Largo, FL is seeking an Intensive Case Manager for the Baby CAT program within the Families First Division. This professional role delivers Wraparound services to families with children ages 0-5, in home, school, and community settings, focusing on stabilizing the family and keeping children safe at home.

You will lead an IDT with a counselor and facilitator, develop individualized plans, address trauma, coordinate with community partners, and ensure rigorous

Qualifications

• Bachelor’s degree in Psychology, Social Work or related area of study.
• Minimum of two years’ relevant experience with at-risk populations.
• Targeted Case Management Certification required.
• Wraparound Facilitator certification within 3 months of employment.
• Excellent writing, communication, organizational and time management skills.
• Early Childhood Development experience preferred.

Responsibilities

• Provide Intensive Case Management for Baby CAT in home, school, and community.
• Develop individualized, strength-based treatment plans with the IDT.
• Collaborate with counselor, IDT facilitator and family to meet treatment goals.
• Ensure trauma-informed, culturally and linguistically competent services.
• Report progress and barriers at IDT staffings and maintain thorough documentation.
