Job summary

Directions for Living is seeking an Intensive Case Manager for the Baby CAT program in the Families First Division. You will provide wraparound case management in home, school, and community settings, coordinating with a counselor and IDT to develop strength-based treatment plans addressing family safety and mental health needs.

The role requires a Bachelor’s in psychology or social work, a Targeted Case Management Certification, and Wraparound Facilitator training within 3 months.

Qualifications

• Bachelor’s in Psychology or Social Work from an accredited university (min 2 years’ relevant experience).
• Targeted Case Management Certification required.
• Wraparound Facilitator certification within 3 months of employment.

Responsibilities

• Provide Intensive Case Management for Baby CAT within Families First Division.
• Deliver services in home, school, and community with the IDT team.
• Develop individualized, strength-based treatment plans with the IDT.
• Report progress and barriers at IDT staffings; address safety concerns.
• Work on-call after hours and weekends; ensure 1410 annual client-facing hours.
