Eliot Community Human Services, Inc. seeks an Intensive Case Manager to join our Adult Community Clinical Services (ACCS) team. You will provide side-by-side, person-centered interventions to adults with serious mental illness, collaborating with a multidisciplinary team.

This role includes a Monday–Friday schedule with rotating weekend and evening on-call coverage, bilingual candidates may receive a salary increase, and a strong commitment to growth, belonging, and community impact.

Formación

• Bachelor’s or Master’s degree in Psychology, Social Work, or related field
• Experience working with adults with mental illness

Responsabilidades

• Provide community outreach to each individual as part of the multi-disciplinary team.
• Offer side-by-side interventions that focus on problem-solving, skill training, modeling, optimism, and encouragement.
• Collaborate with the team and each individual to develop a person-centered treatment plan.
• Support individuals in becoming fully integrated into their communities by helping them build natural supports
• Empower individuals to take control of their lives, focusing on their treatment, leisure activities, employment, and personal recovery
• Provide on-call coverage, as part of a rotation, during evenings and weekends
