The Department of Health and Human Services in Maine is seeking a full-time intensive case manager to support high-risk mental health clients at the Maine State Prison in Warren, with occasional travel to Augusta or Rockland. A state vehicle is provided for travel.
You will work closely with internal and external professionals to facilitate access to medical, psychiatric, behavioral health, and housing resources, and to assist with discharge planning within a coordinated care framework.

This is a full-time position based in Augusta or Rockland and requires up to three days per week at the Maine State Prison in Warren. A state vehicle is available for this travel.
Currently, this position is not eligible for visa sponsorship or STEM OPT extensions.
Education, training, and/or experience in social or behavioral science, mental health, or another closely related field demonstrating competency in one or more of the following areas: assessing behavioral or mental health issues; social work; case management; identifying and coordinating community or clinical resources; interacting effectively with high-risk individuals. Some positions may require a Licensed Social Worker (LSW) license as issued by the Maine State Board of Social Worker Licensure or certification as a Mental Health Rehabilitation Technician - Community (MHRT-C) issued by the Muskie School of Public Service.
