Job summary

Pathways seeks an Intensive Case Manager to engage clients in implementing individualized recovery plans and connect them with essential community services. You will monitor progress through regular visits and coordinate with doctors, residential providers, and other services to reduce crises and improve housing stability and employment outcomes.

Qualifications

  • Bachelor's degree in Psychology, Social Work, or a related field.
  • Professional communication and sensitivity to culturally diverse populations.
  • Valid Georgia Driver’s License and clean MVR.
  • Clean Criminal Background Check (no conviction or credible evidence of abuse, neglect, or sexual exploitation).

Intensive Case Manager
About your Role

As our Intensive Case Manager, you will engage the client in implementing an Individual Recovery Plan (IRP), based on their self‑identified goals and objectives, service needs, strengths, barriers, support systems, and emotional triggers. Using this plan, you will provide support and training, and link the client to community services to assist with identified needs. Through regular community visits, you will monitor the client’s progress and need for additional supports to decrease hospitalizations, incarcerations, and episodes of homelessness, and increase their housing stability and employment and community engagement. This role works with clients’ doctors, residential providers, and other services involved to provide wrap around support.

What we are looking for
  • Bachelors’ degree in Psychology, Social Work, or a related field
  • Professional and courteous communication (verbal and in writing) and knowledge of and sensitivity to culturally diverse populations.
  • Valid Georgia Driver’s License and clean Motor Vehicle Record (MVR);
  • Clean Criminal Back Ground Check (no conviction or credible evidence of abuse, neglect, or sexual exploitation-child or adult)
