Job summary

Esperanza Health Center in Philadelphia is seeking an Intensive Care Manager to work with our medical staff and behavioral health professionals to provide comprehensive case management for patients seeking substance use support.

The role emphasizes culturally sensitive, trauma-informed care within a SUPHR framework, collaboration with care teams, and linkage to MAT where appropriate. A strong commitment to serving underserved communities is essential.

Qualifications

• Must meet one of the following: Master's degree in Social Work, Counseling, Psychology, or related behavioral health field; pursuing LSW licensure.
• Or a Bachelor's degree and active enrollment in a qualifying Master's program in the listed disciplines.

Responsibilities

• Receive and manage patient referrals and facilitate initial screening and intake.
• Provide ongoing care management using culturally sensitive, trauma-informed approaches.
• Develop individualized care plans addressing identified needs and goals.
• Connect patients to culturally appropriate resources and external referrals as needed.
• Monitor recovery progress and communicate with the care team regularly.
• Adjust care plans in collaboration with patients and teams; refer to higher levels of care when indicated.
• Provide crisis intervention and safety planning as appropriate.
• Document all progress accurately in the EMR.

Location: K&A Clinic — Kensington & Allegheny

Salary Range: $50,000 – $57,000 annually (commensurate with degree, licensure, and experience)

Status: Full-Time, Exempt

REQUIRED QUALIFICATIONS

JOB SUMMARY

The Intensive Care Manager (ICM) works alongside our medical staff and licensed behavioral health professionals to provide comprehensive case management services to patients seeking support with substance use. Services include referral and linkage, individualized care management, and support for adherence to available treatments, including Medication Assisted Treatment (MAT). The ICM practices within a Substance Use Prevention and Harm Reduction (SUPHR) framework using culturally sensitive, strength-based, and trauma-informed approaches.

WHAT YOU WILL BE DOING

• Receive and manage patient referrals; collaborate with departmental staff to facilitate and expedite initial screening and intake.
• Provide ongoing support and care management to individuals engaged in substance use, utilizing a culturally sensitive, strength-based, trauma-informed approach to build and maintain a therapeutic alliance.
• Collaborate with patients and care teams to develop individualized care management plans that address identified needs and support progress toward patient-identified goals.
• Identify and connect patients to culturally appropriate resources across EHC and the broader community; provide referrals to outside agencies as appropriate.
• Monitor and evaluate each patient's recovery progression; communicate progress to the care team regularly and flag concerns as they arise.
• Address and amend care plans as needed in collaboration with patients and care teams, utilizing clinical assessment skills to match patients with the appropriate level of care, including referrals to higher levels of behavioral health care when indicated.
• Provide crisis intervention, crisis management, and safety planning as appropriate.
• Document all patient progress and services provided accurately and in a timely manner in the Electronic Medical Record (EMR).

PHYSICAL DEMANDS AND WORK ENVIRONMENT

• Ability to sit, stand, and move throughout the clinic for extended periods
• Ability to use a computer and telephone for extended periods
• Occasional travel between Esperanza sites as operationally needed (less than 10%)
• Ability to remain composed and professionally effective in high-stress situations, including crisis
• Ability to lift and carry materials up to 15 lbs. as needed
