Qualifications

• High School Diploma required.
• One year of experience in behavioral or mental health preferred.
• Understand, advocate for individuals' rights.
• Strong de-escalation and crisis management skills.
• Use of standard equipment to perform essential functions.

Responsibilities

• Complete mental health and functional assessments; identify strengths and needs.
• Assist individuals with exploring hopes, dreams, and goals.
• Support participation in person-centered service planning process.
• Execute individualized service plan centered on the person’s needs.
• Collaborate with professionals to facilitate progress toward treatment goals.
• Engage in planning and communication with support resources.
