Job summary

William F Ryan Community Health Cen is seeking a Mental Health Provider to deliver mental health services including outreach, assessments, counseling, psychotherapy and case management in an integrated health care setting. The role involves collaboration with interdisciplinary staff and coordination with community agencies to ensure comprehensive patient care.

The position requires a Master’s degree in a relevant field and NY licensure (LCSW/LMSW/LMHC).

Qualifications

• Master's degree in a field aligned with social work or psychology.
• Current NY licensure as LCSW/LMSW or LMHC required.
• Experience with electronic medical records (EMR) systems such as eClinicalWorks.
• Two years of health/behavioral health psychotherapy experience preferred.
• Bilingual: English/Spanish preferred.
• CASAC certification is a plus.

Responsibilities

• Provide crisis intervention and psychotherapy (individual, group, family).
• Screen and evaluate patients referred to the mental health department.
• Treat child, adolescent, young adult, or adult populations as appropriate.
• Collaborate with the treatment team and community agencies to coordinate care.
• Determine appropriate referrals and advocate for patient services.
• Maintain documentation and conform to departmental policies.
