Job summary

Sanford Health in North Fargo is seeking an Integrated Health Therapist to join our PCMH primary care team. The IHT provides behavioral health services, including crisis assessment, counseling, and telehealth care to a diverse patient population within ambulatory primary care settings.

Qualifications

• Master's degree in Counseling, Social Work, Psychology, or related Behavioral Health Sciences required; license must be in good standing.
• In North Dakota, must be independently licensed as LPCC or LCSW.

Responsibilities

• Provide behavioral health services in a primary care clinic, including diagnostic assessment, crisis assessment and intervention.
• Deliver evidence-based psychotherapeutic services using multiple modalities (including telehealth) as appropriate.
• Coordinate with the PCMH/PCC team to address psychosocial and cultural factors affecting health and wellness; refer to specialty care when needed.
