Job summary

The LPN will provide nursing services, health monitoring, and assessment, while coordinating medical care with physicians and other providers.

Qualifications

• Graduate of an accredited Practical Nursing program.
• Current Delaware LPN license.
• Valid driver’s license with acceptable driving record.
• Knowledge of medication administration and care coordination.
• Strong communication and documentation skills.
• Ability to work effectively within a multidisciplinary team.

Responsibilities

• Provide nursing services and health monitoring.
• Assess and respond to health concerns.
• Coordinate medical care with physicians and other providers.
• Administer medications per physician orders.
• Maintain accurate, timely, and confidential records.
