Job summary

Ohel Children's Home and Family Services seeks a Care Coordinator to manage care for adults enrolled in Ohel’s Health Home program. You will assess physical, mental health, and social service needs and develop an integrated plan of care in collaboration with medical, behavioral health, educational, and social service providers.

You will provide care coordination, health promotion, transitional care and follow up, referrals to community resources, and use health IT to link services.

Qualifications

• Bachelor’s degree in Human Services required.
• Master’s degree preferred.
• Experience with behavioral health or substance use disorders a plus.

Responsibilities

• Assess physical, mental health and social service needs of adults.
• Develop an integrated plan of care with providers.
• Coordinate care, promote health, and link services via health IT.
