Vanna Health is hiring Integrated Health Coaches to support individuals with behavioral health challenges in Queens and surrounding areas. Coaches identify members, build trust, and provide coaching, advocacy, and care coordination across home visits and remote work.

Rate depends on language proficiency (Spanish fluency at $29/hr)

Work Model: Hybrid — home-based and community-based. Coaches complete remote responsibilities from home and travel regularly throughout Queens to meet members in their homes and communities.

Coaches are responsible for locating and engaging members, building trusting relationships, and providing exceptional coaching, advocacy, and support throughout their recovery journey.
This partnership begins with understanding each member’s personal goals and connecting them to the right resources to achieve those goals. Coaches play a critical role in both physical and behavioral health coaching, collaborating closely with clinicians and specialists to ensure members receive coordinated, high‑quality care.
