Qualifikationen

• Graduate of an accredited school of Pharmacy.
• Texas Registered Pharmacist license; sterile product certification required if sterile compounding.
• Two years hospital pharmacist experience; familiarity with unit-dose and IV admixture.
• PGY1 residency highly desired; PALS certification preferred.

Aufgaben

• Fills and labels prescriptions; provides drug information to patients and staff.
• Dispenses controlled substances; maintains inventory records.
• Supervises pharmacy technicians and mentors pharmacists or residents.
• Participates in Medication Use Evaluation and Process Improvement programs.
• Maintains medication profiles; reviews for drug-related problems; consults on drug research.
• Prepares IV solutions and additives; ensures proper labeling and dosing.
• Initiates reports of medication errors or adverse drug interactions.
• Assumes department management in the absence of the Pharmacy Manager or Lead Pharmacist.
• Drafts policies for review by Pharmacy Director.
• Performs clinical interventions (dose adjustments, IV to PO conversions, antimicrobial stewardship) and documents outcomes.
