Job summary

AbsoluteCARE Medical Center & Pharmacy seeks a licensed clinician to provide comprehensive case management for members with complex medical, behavioral, and social needs. The CCM will assess, plan, and coordinate care across providers, plans, and resources, with telehealth and home/community visit flexibility to engage members.

The role requires travel to meet members where they are, maintain PCCPs, and deliver education and interventions to achieve value-based outcomes.

Qualifications

• Licensed clinician (RN, LCSW, LMSW, LMHC, LPC) in the practicing state.
• Preference for CCM credentials or 3+ integrated case management experience.
• Experience with complex populations and strong documentation/communication.

Responsibilities

• Attend member visits with follow-up support for care coordination.
• Complete comprehensive assessment and PCCP for each member.
• Develop and maintain PCCPs using SMART goals.
• Educate members and perform teach-back on conditions and options.
• Collaborate with ICT and coordinate services with providers and plans.
• Document interactions within 1 business day and meet KPIs.
