Job summary

Community Health Resources in Manchester, CT is seeking a dedicated BHH Specialist to join our Behavioral Health Home team.

In this full-time role, you’ll coordinate care for adults with complex medical and mental health needs, provide comprehensive care management, and support patients and families.

Remote day potential and mileage reimbursement are available; compensation starts at $19.28/hour, with a full benefits package.

Qualifications

• BA in human services required.
• Two years experience in mental health setting required.
• Bilingual candidates strongly preferred.
• Valid driver’s license and CPR/First Aid Certification within 6 months of hire.

Responsibilities

• Comprehensive Care Management.
• Care Coordination and Health Promotion.
• Comprehensive Transitional Care and Patient/Family Support.
• Referral to Community Support Services and collaboration with BHH team.
• Assist in hospital discharge processes to promote coordinated care transitions.
• Maintain documentation in compliance with agency standards.

Skills

Two years experience in mental health

Bilingual candidates preferred
