The Integrated Care Social Worker is a key member of the interdisciplinary care team at El Centro de Corazon, responsible for delivering mental health support, psychosocial assessments, and community resource navigation. This role is central to the clinic’s integrated care model, ensuring that behavioral health and medical services are coordinated to improve patient outcomes. The social worker actively participates in warm hand-offs to support real-time patient engagement and continuity of care.

RESPONSIBILITES AND DUTIES

• Conduct comprehensive psychosocial assessments for patients referred by medical providers and/or dental providers.
• Diagnose common mental health conditions and deliver brief, solution-focused interventions using evidence-based therapeutic modalities such as:
• Cognitive Behavioral Therapy (CBT)
• Behavioral Activation
• Motivational Interviewing (MI)
• Actively participate in warm hand-offs by engaging patients during or immediately after medical visits to ensure timely behavioral health follow-up.
• Collaborate with medical, behavioral health, case management, and care coordination teams to develop and implement patient-centered care plans, follow up on referrals, and address social determinants of health.
• Facilitate referrals to internal and external programs intended to address specialty mental health, substance use treatment, and social service support.
• Contribute to interdisciplinary case reviews and integrated treatment planning.
• Educate patients and families on coping strategies, treatment adherence, and available resources.
• Maintain accurate and timely documentation in the electronic health record (EHR) in accordance with clinic policies and regulatory standards.
• Support clinic-wide initiatives related to trauma-informed care, health equity, and social determinants of health.
• Ability to work full-time, onsite work schedule.
• Maintain productivity as set forth in individual employee agreement.
• Other duties as assigned and abide by agency’s policies and procedures including the Behavioral Health Department policies and procedures.
• Master’s degree in Social Work (MSW) from an accredited institution.
• Licensed Clinical Social Worker (LCSW) in the state of Texas.
• Minimum two (2) years’ experience working in integrated care, primary care, or community health settings preferred.
• Strong knowledge of behavioral health, case management, and local community resources.
• Proficiency in brief therapeutic interventions and evidence-based practices.
• Excellent interpersonal, communication, and crisis intervention skills.
• Commitment to serving underserved and diverse populations.
• Bilingual proficiency in written and spoken English and Spanish required.
