Job summary

Jobtailor is recruiting a Care Manager to provide integrated whole-person care management across physical health, behavioral health, I/DD, LTSS, pharmacy, and unmet health-related needs. You will complete member assessments, identify barriers and strengths, educate members and families, and collaborate with multidisciplinary teams to develop, review, and implement personalized care plans.

Qualifications

Bachelor’s degree in health, psychology, social work, nursing or related field.
RN licensure or CCM certification preferred.
Residence within NC or within 40 miles of the NC border.
Excellent communication skills; bilingual preferred.
Knowledge of care management principles and regulations.
Proficient with EMR and MS Office.

Responsibilities

Provide integrated whole-person Care Management across physical health, behavioral health, I/DD, LTSS, and needs.
Complete member assessments including biopsychosocial and cultural needs.
Identify barriers and strengths with members and families; coordinate care.
Educate members and families about diagnoses, meds, resources, and self-management.
Set SMART goals and monitor quality and effectiveness of interventions.
Develop, review, implement, and evaluate the member care plan with the team.
Document in the care management system per policy; maintain privacy compliance.
