Integrated BH LICENSED CLINICAL SOCIAL WORKER, LCSW

Under the supervision of a licensed mental health or medical professional, the Licensed Clinical Social Worker (LCSW) provides services to patients within Wayne Memorial Community Health Center's primary care clinics. The LCSW is an integral team member delivering a variety of services to patients referred by a primary care provider.

The LCSW engages in population-based education, prevention, and early intervention, participates in chronic disease management, and conducts psychosocial assessments to identify emotional, social, and environmental factors affecting patients' health and well-being.

Job Requirements

• Must be a Licensed Clinical Social Worker in Pennsylvania. No equivalent credentials will be considered.
• Master’s Degree from an accredited school.
