Job Summary

The Integrated Behavioral Health Specialist (IBHS) supports and incorporates the Frederick Health (FH) mission, vision, core values, and customer service philosophy into their job performance. They adhere to the FH Compliance Program, including all regulatory requirements and the FH Standards of Behavior. The IBHS provides quality behavioral health care to individuals in need of psychosocial support and assists them in finding the appropriate modality of care through efficient and integrated therapeutic resources, including hospital-based programs and referrals to local agencies and private practitioners.

Responsibilities include:

• Coordinate behavioral health services such as psychosocial assessment, diagnosis, counseling, referrals, education, and support to patients and families.
• Establish individualized treatment plans with measurable goals.
• Provide crisis counseling and referrals to higher levels of care as needed.
• Collaborate with primary care providers, care managers, or community partners.
• Advocate for patients and their families.
• Maintain accurate records in the EHR and communicate appropriately with caregivers.
• Coordinate discharge plans and referrals, ensuring follow-up and linkage with subsequent providers.

Knowledge, Skills, and Abilities:

• Effective interpersonal and communication skills.
• Ability to maintain confidentiality of patient and employee information.
• Working knowledge of electronic health records.

Minimum Education, Training, and Experience:

• Master’s degree in social work from an accredited school.
• Qualification for licensure or certification as MHC, LCPS, Clinical Professional Counselor, or Professional Counselor.
• Maryland LCSW-C licensure or equivalent (MHC, LCPC, LSCW-C, or Clinical Professional Counselor).
• CPR certification.
• At least 2 years of supervised clinical experience in mental health counseling post-degree.
