Qualifications

• Master’s degree in social work, counseling, psychology or related field.

• Experience in outpatient mental health and/or substance use settings is desirable.

Responsibilities

• Provides direct integrated co-occurring behavioral health and substance use services within primary care.
• Creates and updates integrated treatment plans and coordinates with the care team.
• Conducts risk assessment and crisis interventions as needed.
• Maintains thorough documentation in the Electronic Health Record and ensures accessibility for the care team.
