Job summary

Community Bridges, Inc. (CBI) is seeking a Psychiatric Mental Health Nurse Practitioner to join the inpatient and outpatient behavioral health team in Arizona.

The practitioner will conduct urgent psychiatric and addiction medicine evaluations, coordinate admissions and discharges, and participate in rounds with medical staff to ensure integrated care for adults 18 and older with complex co-occurring conditions.

Responsibilities

• Completes urgent psychiatric and addiction medicine evaluations for 23-hour observation unit patients.
• Coordinates admitting, transferring, or discharging patients with the care team.
• Ensures daily patient evaluations and timely discharge planning updates.
• Writes and documents orders for restraints per policy and coordinates with nursing staff.
• Participates in rounds and discharge planning with the medical team.
