Job summary

Northland Health Centers is seeking a compassionate Licensed Clinical Social Worker (LCSW) to join our integrated Behavioral Health team.

Working under the Director of Behavioral Health, you will provide evidence-based outpatient counseling for patients with mild to moderate mental health concerns while collaborating closely with physicians, nurse practitioners, physician assistants, and the primary care team to deliver coordinated, patient-centered care.

Qualifications

• Current North Dakota licensure as a Licensed Clinical Social Worker (LCSW).
• Knowledge of evidence-based counseling and behavioral health treatment modalities.
• Excellent communication, documentation, and interpersonal skills.

Responsibilities

• Provide evidence-based individual counseling and psychotherapy.
• Complete behavioral health assessments, treatment plans, and clinical documentation.
• Collaborate with primary care providers regarding patient care and treatment recommendations.
• Identify social determinants of health and connect patients with community resources.
• Coordinate referrals for psychiatric evaluation, specialty behavioral health services, crisis intervention, or hospitalization when appropriate.
• Promote patient education, wellness, and self-management strategies.
